Clutch release bearing failure

BMW X1 Manual petrol 2018, 49k miles
Difficulty selecting gears, particularly on change down to low gears. RAC took car to garage, they diagnosed clutch failure. They replaced the clutch and DM flywheel.
After 4 weeks the clutch failed again but this time catastrophically.
Took it to a different garage, they found the Clutch Release Bearing burnt out, it was actually melted into the Release fork and the fork had spun up crashing into the clutch housing.
Trying to work out what happened, was it something the first garage did wrong? Perhaps a mis-alignment?
Anyone else seen this?
