Someone definitely tried to use the Torx socket on top - you can see the nicks in the *clockwise* direction. Would not take much to damage the plastic internally. Just look at the "5+2Nm" torque spec right on it!
If you can find the part, I'd just replace the whole cap. But you could maybe glue the insert.
