When the engine has warmed up, does it still not push air through the upper vents? I know on 'auto' mode the car only sends warm air to footwells until the engine is warmed up, then I think it sends warm air through upper vents also. Maybe same in manual mode?
Also it's probably obvious, but have you checked the dials next to upper vents are open?
