Both.
If it's wider, but same offset, then you add half the extra width on each side of the centre-line.
If it's bigger offset, but same width, then you add the extra offset on a single side of the centre-line.
If it's wider AND bigger offset, then you have two lots of extra to add.
...of course, don't forget to factor in the tyre you're putting on, as some (of the same published size) are wider/taller/narrower/smaller than others.
